Comparison of Inflammatory Cytokine and Growth Factor Expression in Two Degenerative Intervertebral Disc Diseases; Herniated Nucleus Pulposus and Spinal Stenosis

Abstract
Background: To investigate the expression of cytokine and growth factors in disc specimens obtained from patients with herniated nucleus pulposus (HNP) and spinal stenosis (SS).

Methods: Western blot assay and magnetic resonance image analysis were performed to evaluate the expression levels of cytokines (IL-1 beta, IL-6, IL-8, IL-12, TNF-alpha and alpha-tubulin) and growth factors (NGF, VEGF, PDGF, TGF, FGF and alpha-tubulin) and the levels of disc degeneration (Thompson¡¯s classification).

Results: All the cytokines and growth factors were tend to be greater in the HNP group than in the SS group (P < 0.05). Among them, NGF (nerve growth factor) , VEGF (vascular endothelial growth factor) and IL-6 were statistically significantly greater in the HNP group than in the SS group (P < 0.05). The level of IL-6, IL-8 and IL-12 significantly increased along with high grade of Thompson¡¯s classification (P < 0.01).

Conclusions: The results of this study correspond appropriately to that HNP progress acutely or subacutely, but SS progress chronically. The pathological mechanism to develop pain in two diseases may be different on cellular level. These findings are supportive data for investigating the process of disc degeneration.
